投标人须按规定格式提交业绩表，并提交相关业绩证明文件。
业绩证明文件包括但不限于：1）销售合同复印件（含相关技术附件）和2）到货验收材料。投标人所提交的业绩证明文件必须至少体现以下内容：合同签署时间、合同签署页（应有双方盖章）、货物名称：（略）

业绩要求2：2018年1月1日至投标截止日（以合同签订时间为准），投标人所投HIPPS球阀的制造商应具有至少2台在HIPPS系统中的供货业绩且技术规格满足1台尺寸不低于18吋、压力等级不低于1500磅，以及1台尺寸不低于22吋、压力等级不低于900磅。或者，投标人所投HIPPS球阀的制造商应具有至少1台在HIPPS系统中的供货业绩且技术规格满足尺寸不低于22吋、压力等级不低于1500磅。上述阀门密封形式均为硬密封的技术要求。业绩证明文件包括但不限于：1）销售合同复印件（含相关技术附件）和2）到货验收材料。投标人所提交的业绩证明文件必须至少体现以下内容：合同签署时间、合同签署页（应有双方盖章）、制造商名称：（略）
